Threshold IMT uses a flow-independent, one-way spring valve: the patient must generate the set pressure before the valve opens, so the load stays the same however fast they breathe. Resistance is dialled in 2 cm H₂O steps on an alcohol-resistant scale, with no pressure gauge needed. It works in any position, with a mouthpiece or a mask, and is used under clinical direction in pulmonary rehabilitation and respiratory physiotherapy.

What is the difference between Threshold IMT and Threshold PEP?
Threshold IMT loads inhalation (9–41 cm H₂O) to train the breathing muscles. Threshold PEP loads exhalation (5–20 cm H₂O) to help open the airways and clear mucus. They look similar but have different uses.
How is the resistance set?
Turn the dial to the pressure prescribed by the clinician. The scale runs from 9 to 41 cm H₂O in 2 cm H₂O steps, with an accuracy of ± 2 cm H₂O.
How long does it last?
Philips states that accuracy and reproducibility are assured for at least two years, while the 2024 consumer leaflet recommends replacing the trainer after one year of use.
Can it be used with oxygen?
Philips lists an oxygen adaptor (HS730A) for Threshold devices. Use with supplemental oxygen should follow the clinician's instructions.
